Observations

To record and store your observations and responses we will use the KSU Survey system.  Please type all responses in the boxes below.  When you have finished you will have the opportunity to save your responses as a PDF file.

Notes on recording your responses 

We will record your responses with an existing system that will appear in a "frame" on the web page.  You should see this system displayed below with a purple banner.  You will be able to record your information in the response boxes and sometimes as multiple choice responses.  Within the response frame you will have more than one page.  You can move forward in these pages by using the arrow button in the lower left corner of the Response Frame.  This button will appear when you scroll to the bottom of the response frame.  On the last screen you will be able to save a PDF file of your responses.  We highly recommend that you save each one for future reference and study.  After you have finished with the response frame, click Next Page at the bottom of the screen.
